You need to have Adobe Acrobat Reader installed on your computer.
This program usually comes with your computer but if you don't have it, 
click here to get it (free).
Here are the forms:

2. Patient Health History Form
When you have the form open in the new window, click on the printer icon or use the "File" then "Print" commands on the top menu of that window.
Once you have the forms printed, please fill them in before you come. (Don't forget to bring them along with you!)
The forms cannot be filled in online.
